We have prepared this patient information brochure to help answer some of your most frequently asked questions.

On your first visit to our office, our receptionists will ask you to complete a patient information form.  You will need to bring a copy of your current insurance card along with all your medications.  Be sure to always bring your medication with you to each visit so that we may have an accurate list.  Your will be asked questions to provide Drs. Katzenstein, Pedone, and Yandel with essential data on your health and medical history.  We will also obtain any medical records pertaining to your heart or present illness.
